At-Home Services

At-Home Services is an array of services for seniors, their families and caregivers which offers options as they learn to cope with the many aspects of the aging process.

Services give choices, offer support and help families in managing care for a loved one while offering information as  decisions about care for older adults. Our goal is to help seniors maintain independence while also giving options and opportunities.

Alternatives Program-

The Alternatives Program (TAP) provides in-home services to medically frail seniors who qualify and are at risk for premature placement in a nursing home.

The program’s goal is to help keep frail adults in their homes by providing and coordinating a variety of services. Each senior’s needs are first assessed by a social worker along with a nurse who then develop a personal care plan to address all needs. The goal is to preserve ech persons resources and allow them to age in place where they are more comfortable.

 Care Giver Support-

Like any journey, the road to care-giving requires a plan, sufficient funds, a sense of humor, and a reliable source of information when we run out of gas or just don’t know where to turn.

The Caregiver Support Program provides limited in-home respite and assistance services as well as various resource and emotional support services to help those tasked with providing care to family and others.
